Uso de fármacos nefroprotectores y restringidos en pacientes con diabetes tipo 2 y deterioro de la función renal que acuden por primera vez a consulta de bajo costo

Introducción: La enfermedad renal crónica (ERC) es una complicación microvascular de la diabetes mellitus tipo 2 (DM2), su prevalencia en México alcanza el 44%. Está ligada con el aumento del riesgo cardiovascular por lo que su abordaje, detección y tratamiento temprano es fundamental para reducir la morbimortalidad no solo renal, sino también la cardiovascular, principal causa de muerte de esta enfermedad. Objetivo: Evaluar la prevalencia del deterioro de la función renal y su asociación con el control metabólico, comorbilidades y patrones de prescripción farmacológica en pacientes con DM2 que acuden por primera vez a un Centro de Atención de Diabetes de Bajo Costo (CAD). Material y métodos: Estudio descriptivo, observacional y transversal se incluyeron pacientes con diagnóstico previo de DM2 que acudieron por primera vez al CAD en el noreste de México entre 2019 a 2022 y con laboratorios de glucosa plasmática en ayuno (GPA), HbA1c, creatinina sérica (cr), colesterol total (CT), C-LDL, C-HDL y triglicéridos (TG). La muestra final fue de 908 pacientes. Para estimar la tasa de filtrado glomerular (TFG) se utilizó la calculadora CKD-EPI con la clasificación de KDIGO. La prueba estadística utilizada fue Chi cuadrada con una significancia de 0.05. Resultados: Únicamente el 4.6% de los pacientes refirió tener el antecedente de ERC. Sin embargo, la prevalencia de reducción de la función renal con eTFG <90 ml/min/1.73 m² fue de 29.6%, mientras que el deterioro significativo de la función renal (eTFG <60 ml/min/1.73 m²) fue de 7.8%. En este último grupo, el 73.4% presentó descontrol glucémico. El uso de iSGLT2 y agonistas GLP-1 fue de 16.9% y 1.4%, respectivamente, mientras persistía el uso de fármacos potencialmente inapropiados según función renal. Conclusiones: El presente trabajo refuerza la necesidad de identificar el deterioro de la función renal de forma más temprana y ajustar los tratamientos farmacológicos, priorizando el uso de nefroprotectores y limitando aquellos que están claramente contraindicados en esta población.
